Position Summary:
The Part-Time 20 hour per week Colorectal Health Patient Care Coordinator encourages and facilitates screening, case management, and patient self-management for colorectal health, and provides motivational lifestyle counseling interventions to patients aged 50-64 regarding the primary prevention, early detection, and self management of chronic diseases for services covered under SEARHC’s CDC grants, with an emphasis on colon cancer. This position oversees the CRC Tracking Package maintenance and its effective use in substantially increasing colon cancer screening rates in the SEARHC population. The goal of this program is to effect change in current systems to raise the colon cancer screening rate among 50-64 year olds to 80% by 2014.
 
Baseline Qualification Requirements:                                                                                                                  
*Graduate of an accredited school of nursing with current RN license.
*At least 2 years experience as licensed RN, with lifestyle counseling or health education services in a public health or ambulatory care setting, includes at least six months experience working within in a clinical setting, handling patient charts, and working within complex systems of patient care and referral.
*CPR training with up-date every 2 years.  Continuing education required to maintain RN licensure.
*Training in motivational interviewing and coursework in health education for chronic disease prevention and management, behavioral risk factor assessment, nutrition, physical activity, or other approved and relevant public health training is preferred and may, if absent, be required after hire.
